How Watsonville Endodontics may use and disclose your protected health information.

Watsonville Endodontics is required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this notice describing our legal duties and privacy practices regarding PHI, and to abide by the terms of this notice while it is in effect.

We may use and disclose your PHI to obtain payment for services we provide to you. For example, we may share information with your dental insurance plan so that it will pay us or reimburse you for treatment.
We may use and disclose your PHI for our own operations to facilitate the function of our practice and as necessary to provide quality care. For example, we may use health information to review the quality of care we provide, or for staff training.
We may also use or disclose your PHI, without your written authorization, in the following situations:
Other than as described above, we will not use or disclose your PHI without your written authorization. This includes, for example, most uses and disclosures of psychotherapy notes (not typically applicable to this practice), uses for marketing purposes, and disclosures that constitute a sale of PHI. You may revoke an authorization at any time in writing, except to the extent we have already acted in reliance on it.
We are required by law to maintain the privacy and security of your protected health information, notify you promptly if a breach occurs that may have compromised the privacy or security of your information, follow the duties and privacy practices described in this notice, and give you a copy of it.
We reserve the right to change this notice and to make the revised notice apply to PHI we already have as well as any information we receive in the future. The current notice will be available at our office and on this page.
